It's endlessly frustrating that we don't view neural adaptation to opioids as a serious iatrogenic harm. On the other hand, as I wrote in 2017 ... https://t.co/Rhol8M7gzC https://t.co/zyRXj2hpbo
RT @DavidJuurlink: @DrChaya About that https://t.co/Rhol8M7gzC
Opioid analgesia attenuates with time, while the harms persist or accrue as doses increase. For some patients, the primary benefit of opioids becomes the avoidance of withdrawal. This constitutes harm, but is easily misconstrued as ongoing effectiveness.

Do they know just how pernicious a side effect dependence is? Do they know it develops within *days*? That it happens eventually to everyone? That it can obfuscate entirely their assessment of whether opioids are helping or harming? https://t.co/Rhol8M7g

@BethDarnall I am in favor of people doing well. But, as has been put eloquently elsewhere 'doing well on opioids' often confers a primary benefit of avoidance of withdrawal. Which then in a real way makes you feel improved when the pill is taken @DavidJuu

@fightpaindaily @esteckler2 I do encourage gradual tapers; rationale explained here https://t.co/Z1wStKNWjk I get that tapering sometimes doesn't work. Re: genetic testing, very limited role IMO. Too many important factors it can't account for.
It somehow ended up that doctors knew how to start prescriptions but fundamentally had no idea how the drugs worked or how to safely withdraw. At that point they are no longer doctors, but glorified drug dealers.

Not true not true not true I have medical records that dispute what you are saying. Why don't you talk of how opiods have helped heal. My personal medical history will show Oxycontin saved my life and helped bring inflammation levels down.
